How to Create Experimental Forms
1. Create manila folders specific to the experiment for the forms you need to save. Label them appropriately.
- This may include folders for consent forms, screening questionnaires, payment receipts, etc.
2. Make subject sheets for the experiment, on which to document subjects as they are run.
- Save them in the folder on the server appropriate to the experiment.
- The template is available under z:\experiments\forms\Subject Recording.xlsx. *Make sure to list the name of the experiment, as well as the consent form it uses. Specific instructions can also be added.
3. Print several of these sheets out and put them on a clipboard. Extra clipboards can be found in the at the bottom of the cabinet in the hallway.
4. Put the stack of manila folders with the subject sheet clipboard.
- For the entire duration of the experiment, the manila folders and clipboard should be kept locked in the cabinet in the hallway for storage.
- (These can, of course, be kept out in the RA room while the RA is actually running experiments.)
5. When the experiment is completely done, and subjects are no longer being run, the subject sheets (sans clipboard) and manila folders should be stored as specified here.
